Quick and convenient health and sports physicals for student athletes. Completed by our licensed provider in compliance with LHSAA requirements. Bring your school form and immunization record. Available in-office or mobile (travel fee may apply).

Your appointment is thoughtfully reserved just for you 🤍 To ensure the highest level of care, please review the following: • A card on file is required to secure your booking • Please provide 24 hours’ notice for cancellations or rescheduling • Changes within 24 hours may incur a $50 courtesy fee • No-shows are charged the full service amount • Arrivals more than 10 minutes late may need to be adjusted or rescheduled Memberships, packages, and promotional services are non-refundable.
